Pay stubs covering the last 30 days (all employers) W-2 forms for the past 2 years Federal tax returns — last 2 years, all pages and schedules Employer contact information If self-employed: 2 years of business tax returns + year-to-date P&L If receiving Social Security or disability: award letter If receiving pension / retirement: award letter Child support or alimony documentation (if part of income)

Bank statements — all accounts, last 2–3 months (all pages) Investment / brokerage account statements 401(k) or retirement account statements Gift letter + donor bank statement (if receiving down payment as gift) Written explanation for any large or unusual deposits Source documentation for closing cost funds

Credit authorization form (provided by lender) Minimum 580 credit score for 3.5% down; 500–579 requires 10% down List of all monthly obligations (car loans, student loans, credit cards) Explanation letter for late payments or derogatory marks Bankruptcy discharge papers and trustee letter (if applicable; 2-year wait) Foreclosure / short sale documentation (3-year waiting period applies)
